Need a sample plugin for creating a combo drop down box

I have gone through the Forum, found a sample plugin but it did work under 7.02.00.
Do you have anything that you can forward to me.

It would be nice to have a VB version, as I have already build all other custom Lookup field using VB.

I've updated the topic with an updated VB.NET version.

All I did was download the existing VB.NET and import it - It complained that it needed a reference to "JiwaServiceModel.dll" and I should add a reference to it. So I did, and it all worked.
